If you think of a glass of water spilled on a table. The table has a cloth on it that has wrinkled up. The water will make little pools in the wrinkles. It will pool in the wrinkles,

In your sentence, it is suggesting that "he" is physically very close to "her" so that his breath metaphorically pools in the wrinkles of her shirt.

No!:-D

To unwrinkle means you are smoothing the wrinkles out of something.

I don't think you really can have a synonym for "pooling" in this sentence. Possible alternative words might be gathered or coalesced, but neither gives the same impression of little pools of something.

Good try - but not quite right. This means blowing into something, whereas his breath merely is trapped in the wrinkles in little pools.
