#1 She is a girl OR That is a girl. Never use "It" for reference to a human.

#2 Yes, you can omit "a" in this question, but it is probably better to have it in.

I heard if a baby's sex is unknown, we use "it".

Except for "identifier it":
"Look, it's Monica!"
"Who's there?" "It's us."

In older English, "it" can sometimes refer to a baby or child of unknown sex:
"In the next room a child began to cry; probably it had fallen down."

In more modern English we would probably try to recast the sentence.
