If I was gonna say,

I had a mistake in the document "showing" A instead of B", or

I had a mistake in the document "showed" A instead of B, or

can I also say I had a mistake in the document showing A instead?

please correct my grammar

No,

'I made a mistake in the document showing A instead.' "Instead" requires instead of <something> here. Or perhaps you could say:
I made a mistake; I should have shown B but I showed A instead.
