he wished he had been rich.

It entirely depends on when he was talking about.

"He wished that he were rich" suggests that the wish took place in the past and that he wished he were rich at the time of wishing.

"He wished that he had been rich" suggests that at the time of wishing, he wished that he had been rich before that time, ie in the more distant past.
