In the phrase surrogacy contract, is surrogacy functioning as an adjective? or is surrogacy contract a noun phrase?

How to tell if a word is functioning as an adjective:

Ask yourself the question, "What kind of...?"

Example:

Q: What kind of contract?
A: A surrogacy contract.

surrgacy functions as an adjective.
a surrogacy contract functions as a noun phrase. It's made up of a determiner (a), a noun (surrogacy), which is functioning as an adjective, and a noun (contract).
