It could mean "vigorously" or "completely."

Add the gin to the ice and shake the hell out of it.
She saw that big old hairy spider, screamed, and stomped the hell out of it.

It could mean to leave or remove yourself from a situation as quickly as possible.
If you're in one of those shady investment scheme, you should get the hell out o fit.
