I would like to know when you use a full stop or semi-colon in a list and/or a capital for the beginning of the line. For example, in writing to a client something along the lines of:
Would you please provide the following:
1. the name and address of your mother;
2. the name and address of your office; and
3. the name and address of your bank.

General Information
1. The name and address of your mother.
2. The name and address of your office.
3. The name and address of your bank.

Is the above correct in both instances?

I look forward to hearing from you.

There is no need for any punctuation after items in a list. Nor do you need capitalization, except for proper nouns.
