1) The manager of the senior home told me that he would leave the form for crimnal background check at the frontdesk for me to pick up.

2) What is the form for or about? (Should I use for or about?).

3) It is the form for doing a background check. (Can I use for in the sentence?).


Did I construct the above sentences correctly. Thanks for your advise.

Sentence three is fine.
