I am wondering if my sentence is grammatically correct.

When Bob left home after a violent argument with his parents, he knew he wasn't going back. The scene played like on a loop in his mind decades after it occurred.

There are two sentences and they are both grammatical.

The second sentence is not well phrased. I suggest you reword it.

Would this version of my second sentence be OK?

The scene played in his mind like on a loop, decades after it had occurred.

You could write like a tape loop or as if it were on a loop. I don't much like the latter but I find it more acceptable than "like on a loop", which doesn't work at all for me.
