1. "fall into" if used in a narration or "fell into" depending on context
2. "has a heart attack" and "Leonard suffers a heart attack" if used in a narration or had/suffered (past tense) depending on context. "Suffers from" would be used if the person continues to live.
3. "goes as planned/designed" if used in narration or "went" depending on context.
