Some of us are suffering from serious life-threatening illnesses. They should firmly maintain their faith in their religion until the last moment, entrust their life/lives to God and continue to pray/praying in their heart/hearts.

I think there is a problem with agreement: "some of us" doesn't work with "they" as I see it. If replace "some of us" with "some people", the answers should be their lives/to pray/their hearts respectively.

Yes. IHIVG is right. The second sentence starts with third person plural as referent to the noun or pronoun mentioned in the previous sentence as some of us which is in first person plural. This is grammatically incorrect and has to be changed to third person plural. The sentence can be rewritten as:
There are some people who suffer from serious life-threatening illnesses. They should firmly maintain their faith in their religion until the last moment, entrust/surrender their lives to God and continue to pray/praying in their hearts.
Or
Some people are suffering from serious life-threatening illnesses. They should firmly maintain their faith in their religion until the last moment, entrust/surrender their lives to God and continue to pray/praying in their hearts.

You can use either an infinitive or a gerund after the verb ‘continue’
