In university, we have a website that assistant and associate professors for
every student enter a number between 0-20 based on their operation in courses.
For example
Mathematics ………………19
Chemistry……………………18
etc.
I have done my project in last semester, but my supervisor forgot to enter my grade operation and I want to remind her for entering it.
What is the correct sentence for writing about it?

I would just write "Please enter my score for the project I did last semester". You might need to remind her what your project was in case she can't remember.
