To be honest, you will hear them all. I think "if asked" is the neatest.
"If he is asked [to]" would be appropriate if the sentence started "He said he will consider ..."
After "would" you could also say "if he were asked" or "if he were asked to".
 
The other good thing about 'if asked' is that it avoids the issue of which tense to use. ;-)
