1. How many courses does a formal meal, like dinner, have when you are in a high-class restaurant?
2. If it has 3 courses, what is it called? 'A 3-dish meal' or 'a 3-course meal' or something else?

I would say that formal dinners can have an unlimited number of courses. In France, at a traditional New Year's Eve banquet, there are up to 13 courses.
However, a standard full meal in the UK would be a three-course meal (starter, main course, dessert).

In the USA, a three-course meal consists of an appetizer, entrée and dessert.

Other courses can be called hors d'oeuvres, soup, fish, cheese and sorbet.
