Are the all structures correct?
If yes then which ones are the most used ones?

In business correspondence (BrE) I imagine that #4 is the most common form.
 
AE speakers typically put the month first.

Once a British person in one of the forums told me that I can also say:
"On the fifteenth of July, he ..."

Is it needed to use article "the"?:?:
I mean can I also say "On fifteenth of July, he.....":?:
 
We say 'On the fifteenth of July'; we write 'On 15th July'.
 
Can we omit the pronunciation of (the) and (of) while reading? Is it still correct if I don't pronounce them?
 
I'm not saying it's wrong, but you're more likely to hear that when it's the other way round.

'On July fifteenth I broke my leg.'

Once I asked this question of a native English speaker. He corrected me very well but I have a problem in one of them.
He told me:

1. On July 15=
(On July the fifteenth) correct
(On July fifteenth) correct - AE not BE
(On the July fifteenth) incorrect

2. On 15th July=
(On the fifteenth of July) correct
(On fifteenth of July) incorrect :?:
(On fifteenth of the July) incorrect

3. On July 15th=
(On July fifteenth) correct - AE not BE
(On the July fifteenth) incorrect
(On July the fifteenth) correct

4. On 15 July=
(On fifteenth of July) incorrect :?:
(On the fifteenth of July) correct
(On fifteenth of the July) incorrect

But I disagree with him on number 4. He has said that "On fifteenth of July" is incorrect. Don't you think that it can be correct too? Can't it be correct even in the USA? (I mean number 4 "On fifteenth of July)

"On fifteenth of July" is incorrect, even in the US.
