Should I use 'this' or 'these' in the following sentence?

1- I have so many health problems. Besides this/these I have no job.

Neither of them leads to a natural sentence. I don't know why you've used "Besides". I think you mean something like "Also" or "In addition". Also, don't use "so many" when you simply mean "many" or "a lot of".

I have a lot of health problems. Also, I don't have a job.
