The man says: "And then "a" older woman came...". Guess it is very common and no big deal for native English speakers, right?

I can't watch the clip at the moment but if the person says "a older woman", it's an error. I wouldn't say it's very common and, for some of us, it's a big deal!

It isn't a slip of the tongue. It's just that when he says a, he hasn't yet planned exactly which word will follow.
