She is suggesting that he is overweight and it would be good if he ate more salad.
 
Exactly. It's conversational shorthand for "[Do you think that] perhaps [you should eat] a salad once in a while?

The idiom once in a while means sometimes, occasionally, from time to time.
 
It is but it's not very accurate - I know plenty of overweight people who eat salad and plenty of thin people who never touch a vegetable!
 
We put French fries on our salads around here. No, I'm not kidding.
