Yours is certainly an improvement on the original which isn't grammatically correct. It could be worded in several ways:

The form in which DOS asks for the time and date may vary ...
The form which DOS asks for the time and date in may vary ...
The form that DOS asks for the time and date in may vary ...
The form DOS asks for the time and date in may vary ...

I would have expected "format", not "form".
