What does the expression "to look somebody dead in the eye" mean? Straight in the eye or something else?

Not only looking straight in the eyes but it also means that a person is ready to stand for his/her words.
Ex. He told her, "though you are suffering of AIDS, still I would marry you, my Blue-eyed honey."
She stared back at him.
And he just looked her dead in the eye.
