It's only you who is/are confused.
Should I use 'is' or 'are'?
Thanks.
Hi Tan
Is the term "you" intended to be singular or plural?

I don't think that that affects the answer. It's a question of whether the subject of the verb in the relative clause is who or its antecedent. I go for 'who' -"It's only you who is confused".
However, many disagree. I would say, "It is I who is leading this project". Many would say, "It is I who am leading..."
On balance, I go for "are".
Hi Tan
It looks like you are not the only one that is confused!
I'm now confused.
