Yes, 'worked' is the past tense of 'works'. All of your phrases could be used in sentences (and they should be).
They mean different things. You use the form that's appropriate.

That depends. What's the sentence in which the phrase is used? If you mean "It worked for me", that's a reference to the past. It's impossible to tell whether anything still works, without more context.

Just to clarify, if I used the treatment in the past, the treatment works and cured my problem, after the problem is solved I did not use it anymore for many years. But I know the treatment still works now.
