I need a clear definition for the above mentioned phrase. what does it exactly mean in the below example?

'Liverpool Street Station. The train terminates here. All change!'

It means "Everybody, change trains."
The train doesn't go any further, so all passengers ('All') need to get off that train, and get onto a different one ('change').

We have explained what it means. It is, if you like, a fixed expression. The only way we can really give you another example is to change the place names:

Thanks for your post, but they seem to be different. I'd searched the aforementioned phrase once in a bilingual dictionary and it has given the phrase 'get off' as the equivalent...as the other precious friends answered.
but what I need was to read it in another original post or article to see if it's a fixed expression or not?
