Alex MacDonald's long-range effort squirmed past home goalkeeper Philippe Hahm on the half-hour before he set up Rhodes for a diving header.

I have some questions concerning this sentence.

1.
Does the first red part of the sentence mean that "Alex got injured and he moved slowly from one side to the other side"?

And does the half-hour mean 60 minutes in the match?

2.
Does the "set up" mean "Alex set up a chance for Rhodes"?

1. "Long-range" means that he kicked the ball from a long way away from its intended target (perhaps he kicked the ball from the far side of the halfway line).

"Effort" as a noun means "an attempt". Here, it's just another word for a kick.

"Squirmed" is unusual here, but I can see that it means it squeezed its way past the goalkeeper (I assume a goal was scored). It wasn't a clean, direct goal - the goalkeeper probably got quite close to the ball but still couldn't save it.

"On the halfhour" means it was 30 minutes into the game.
