Said on the phone, it means they will connect you to the person you are calling.

"Hello, Acme Incorporated. How may I help you?"

"Hello, I'd like to talk to Mr. Smith."

    "I'll put you through now."

I presume that the speaker is a telephone switchboard operator. He or she is telling a caller that the caller will be connected to the person that they have been waiting for.
    Switchboard: Good morning, ABC Construction.
Caller: I'd like to speak to Mr. Smith.
Switchboard: I'm sorry, his line is busy. Would you like to hold?
    Caller: Yes, thank you.
Switchboard (a minute or two later): I'll put you through now.

To avoid any confusion, only one "through" is required.
