    Default Plural Salutations

Hello, can anyone correct/confirm the accuracy of this?

How to refer to two doctors with the same surname (ie: Smith):

    If they are biologically related:
Immediate family (ie: father and daughter): Doctor and Doctor Smith
    Siblings: Doctors Smith
*Would cousins be: Doctor and Doctor Smith or Doctor Smith and Doctor Smith?

    Not related:
Just two people: Doctor Smith and Doctor Smith
    Married: Doctor and Doctor Smith

*Assume for this question's sake that you cannot refer to them by their first names please!

    -Thank you!

That, unfortunately, shuts out the most natural answers for most native speakers.

I find it rather tiresome when native speakers ask this sort of 'How many angels can dance on a pinhead?' question with the sole intention of making life difficult for language learners.
