No, it's definitely not correct. I'm not sure why you want to use causative verbs in that sentence. My suggestion would be, "I read a book that changed my way of thinking." (One of the meaning of the verb, "change" is to make something/someone different. It can mean both "megváltozik" and "megváltoztat", in Hungarian. Do you understand now? )

No. The subject is "The book", so the verb must be "has changed". By changing the subject, you can change the verb: "Things have been changed by the book." or "Things have changed because of the book."

I'd say you have many simpler grammatical tasks to master before forcing yourself to write sentences in the causative form, unless this is a homework task. You'll never get a causative sentence right until you can write a simple sentence like "The book has changed things" using the correct subject-verb agreement.
Both Gillnetter's and ~Mav~'s sentences are good. Here are some more:
"The book made me think in a different way."
"My point of view was influenced/changed by the book."
"The book caused me to re-think my opinions."
