It means that if the young girl had paid Dennis the dollar for the premium service, her balloon would still be whole and the helium would be inside.
Not all balloons are filled with helium, but ones that get stuck on lamp-posts tend to be.

PS A trivial point; he doesn't look like Dennis to me. Dennis has black hair - improbably spikey, in the days before hair-gel. Maybe this is Dennis in his younger years, or Dennis's little brother or something
