I have problems in understanding this rule, my queastions are :

1- when I start queastion by Does and Do?

2- when I strat queastion by Is and Are?

and I have to know when I use (Does or Is ) in queastion for example;

# Is he crazy? OR Does he crazy?

last queastion teach me how to make queastion please like ;

when should I use these at beginning of the question;

what are OR what is OR sometimes without are& is ,like = what ingredients to combine....?

Let's start with your first question:

If we have auxiliary verbs, including modals, or the full verb BE, we do not use DO to form questions.

Can I help you?
Have you seen that film?
Is John making supper?
Are they Russian?

All other full verbs use DO for questions:

Do you understand?
Does Mary speak German?
Did they arrive yesterday?

You start a question with "Is" or "Are" when you are asking about an adjective, or when you use the continuous.

Are you cold?
Is he hungry?
Are they tired?

Are you coming to the party?
Is he going on holiday this year?
Are they visiting you for Christmas?

Please remember to start every new sentence with a capital letter.

Second example if I want ask somebody about somebody else about if he goes to gym or any place what should I say?

So, there are two cases about use Is OR Does OR Are OR Do, if you have more tell me please.

I understand how confusing it is for learners to make questions in English.

As the other posters told you, try to remember this:

If the verb is "to be," then use "to be" in the question:

I am ugly. Am I ugly?
You are intelligent. Are you intelligent?
She/he is rich. Is she/he rich?
It is Monday. Is it Monday?
We are students. Are we students?
They are teachers. Are they teachers?
He was sick. Was he sick?
They were absent. Were they absent?

If the sentence has auxiliary verbs or modal verbs, repeat them:

We will take a test. Will we take a test?
They are eating. Are they eating?
They can speak French. Can they speak French?
It has rained. Has it rained?



In other cases, use "do" or "does" or "did"

I eat. Do I eat?
I ate. Did I eat?
She sings. Does she sing?
She sang. Did she sing?
