Letter to a nursing home

Hello there, I have been trying to verse a letter to a nursing home or assisted living center for a family friend of mine who is all alone and has no family or anyone close to rely on. She wanted me to write a letter to a nursing home(home for the elders) asking to accept her admission since she doesn't have any family and if anything should happen to her, the nursing to do what's needed at the event of her death.
