Signing a letter on behalf of another ???

I used to know this. I think. I can't find the answer on the internet anywyere.
What is the proper format for signing a letter on behalf of another person? For example, a secretary signs for the boss. Does the secretary sign the boss's name "John Smith" and then indicate in some way that it is a proxy signature? Or does the secretary sign his or her own name and then "For Mr. Smith" or something like that?

Usually the secretary will sign the boss' name, then draw a forward slash, and then sign her initials:
John Q. Public/sw
