I need a help in writing a letter to editor of journal

Hi Sir

I need help in writing a letter for scientific journal editor in a kind manner
Would please, help me in writing a sentence with following requests

1- If I want to submit a paper and want from him to send it to the reviewers
2- If I want to ask about the status of my submitted paper where I submitted it since 4 months ago, while i did all modification I responded to the reviewer comments
3- If I want to ask him also to send me a formal letter that depicted acceptance of my article at my postal address
4- If I want to ask him kindly to send the issues that contains my article
