It is has been noted that you have informed over phone to HR that you are availing a leave from 15th April 2008 informed HR by phone that you are proposing to take leave from 15th April 2008 onwards due to sickness of your Brother your brother's illness. However, you have not submitted the necessary written leave application form. This is to inform you that without the completed leave application, we are not able to process your salary.
