You might consider adding Dear Madam, but essentially yes.

I always use 'Sir or Madam' as I think just 'Sir' is not appropriate with so many women in the workforce. 'Madam' alone might be rectifying history, but I wouldn't go for it.

In BE, that would not be the case- we'd use 'faithfully'.
