Default Proper terms of expressions/phrases

Dear Sir,

Could you please give me a proper word or words of the following expressions or phrases, which I have happened to see many times in official documents. The words or phrases given hereunder are made up by me myself which make me uncertain whether they are correct or not. Kindly help me correct them as necessary.
1. Person granting the power of authority: <a person who gives the power of attorney to someone>
2. Signatures of the Appointor/Grantor: &lt;Signature of the person who gives the power of attorney
to someone&gt;
3. Person issuing the card; Officer issuing the card: <an issuer of the card. Which one is correct?>
3. Person knowing about the birth:<a person who happens to know about the birth of someone or
someone giving a birth to a baby: in the case a witness giving a statement to an inquiry officer to
certify that said person was born in that village>
5. Mentor/consultant/ adviser&lt;What the difference among these words?&gt;
6. Mentee's behaviour:&lt;What does it mean?&gt;
7. Name of respondent:&lt;Name of person giving an answer&gt;
8. The person who gives the consent/the person giving the consent
9. Advisor:&lt;teacher who gives advice on his/her students&amp;#8217; educational areas&gt;
10. One who gives false testimony:- a person who makes or provides false witness and evidence
11. one who gives testimony:<a person who makes or provides witness and evidence>
12. Person giving the certification:<a person certifying true contexts of documents>
13. person giving the statement:<a person making a statement>*
14. person giving a consent:<a person who gives a consent to allow someone to do something>
15. Reviewer:<a person review an affidavit before getting it signed by the affiant>
16. Person Assigning the Figures (Numbers) :<a person giving the run No. of the documents>. 17. Recipient of Registration Copy:<a person receiving the copy of house registration>
18. Affiant:&lt;a person giving a statement to the inquiry officer (or a person making an inquiry)
19. Recorder:<a person taking the note of the affiant>
20. A legally-authorized guardian of the minor:<a person who is legally authorized to be the
guardian of a minor >
21. An adviser the a student who is writing his thesis:&lt;What is the proper name of this person?&gt;

That is quite a list. I will give you some words, but please understand that specific words often mean different things in different professions. You would probably be better off asking someone who is active in this particular field.

1. designator, appointor
2. authorization. A person with a power of attorney usually signs his/her name, follwed by "attorney in fact". This distinguishes them from an attorney who is a lawyer.
3. person, official, issuer
4. witness would work, attestor, deponent
5. A mentor is a person who is responsible for the training of someone else. It is usually an intense relationship. An advisor or a consultant is a person who provides advice. The relationship is usually much less intense than a mentoe/mentee relationship.
6. A mentee is someone who is being mentored by someone else.
7. This just asks for the name of the person who is ansering questions.
8. This person would be some authority figure, parent, guardian, employer, etc.
9. An advisor is someone who gives advice.
10. One who gives false testimony (under oath) is a perjurer. This person could also be called a liar.
11. witness, deponent,
12. certifier, certifying authority If it is a signature, it could be a notary public.
13. respondent, deponent, witness
14. already covered
15. reviewer works
16-20 Those don't sem to be questions
21. advisor, major professor, mentor
