What are the ages of your children?
What ages are your children?
How old are your children?

I think the last one is the simplest.

I think they are not correct, both of them. There are no verbs in the sentences.

What's the age of the boy? 's is an aspect of the verb to be
How old your children are? are is an aspect of the verb to be
