If the T is in the middle of the word, intonation changes the sound to a soft D.

In most places, they're pronounced 't'.
There are variations in various regions, but if you're a learner, I can't see any reason not to just pronounce it 't' unless you particularly want to copy a Cockney or American accent.

Excuse my butting in - but hanky's confusion can be easily dispelled.

It must have been a slip - easily explained by the idea of regional pronunciation differences (that is, the idea was in Raymott's mind, so it was an easy slip to make).
