You need to listen as much as you can and to analyse the way native speakers say things. You also need to focus on sounds that you have trouble with and work on those; try doing it in front of a mirror and record youself if possible. Also, remember that the spoken word and the written word can be very different.
I am having trouble in communication skill, What should I do? Sometime, while communicate with someone, I forgot the vocabulary so It hard for me to deliver my message. Have any suggestion, how I can improve my communication skill?

To speak English properly you need to use correct intonation. English is not a tonal language but intonation and rhythm are critical to conveying meaning.
Remember that non-stressed words and syllables are often "swallowed" in English.
2. Always focus on pronouncing stressed words well, non-stressed words can be glided over.
3. Don't focus on pronouncing each word. Focus on the stressed words in each sentence.
