2. Answer those questions based on what you've heard.
3. I've made the decision based on his suggestion.
Are those sentences grammatical and anything you would say in daily life?
I am confused about the usage of "based on".

The first sentence is incorrect.

You can replace "based on" with "using".

To retain 'based on' you could rewrite #1 like this:

Based on the title, to predict what the passage is about.
Are other sentences correct?

I am confused about the usage of "based on".
Some usage authors object to this adverbial use of "based on," finding it to be a dangler. In none of the sentences is there a noun phrase that "based on" modifies.

One easy way around that problem, at least for those who perceive it to be a problem, is to replace adverbial "based on" with "on the basis of":

1a. On the basis of the title, predict what the passage is about.
2a. Answer those questions on the basis of what you've heard.
3a. I've made the decision on the basis of his suggestion.

This is not to say that "based on" is always bad or always a dangler. It is perfectly fine in sentences like these:

"His prediction was based on the title."
"Those answers of his based on what he had heard turned out to be incorrect."
"I regret the decision I made based on his suggestion. I should have gone with my instincts."
