Comma before '...the latest one being...'

I'm struggling write the below paragraph without it sounding clumpy to read and wonder if someone could offer some assistance.

"We note from our records that you have failed to attend an appointment with us on more than one occasion, with the latest one being Friday 7th October 2016 at 10am."

The part I feel doesn't read as well as it possible could is '...with the latest one being...'. Is a separate sentence required or will a comma suffice? Also, would it be perfectly normal not to include 'with the latest' and simply write 'the latest one being'?

Yes, you do not need "with" after the comma.

Are you sure that's what you meant? Did you perhaps mean "clums​y"?
