A) How he helps others says a lot about him.
B) It says a lot about him how he helps others.

Do these sentences mean:
1) The manner in which he helps others says a lot about him.
or:
2) The fact that he helps others says a lot about him.

I think 'A' is ambiguous and 'B' could only mean '2'. But I am not sure at all.

I find sentence B unnatural.
 
I'm not sure what you mean by "The way he helps others". Are there good ways and bad ways to help people?

P.S. I agree that there are good ways and bad ways to help people, but how or even if I would say that would depend on context.
 
"The fact that he helps others ..." would be better expressed as "That he helps others ..."
 
Only A works for me, and could mean either.
