Is that sentence grammatically correct?

No. The first part needs a couple of commas and has, I suspect, an extraneous word that makes the second part nonsense. Do you mean this?

It represented service income provided by the director, based on the actual cost incurred by him, with a 5% markup charged to the six companies in the group.
