The question says

She had extra groceries at home. She ..... Any more
A: didn't have to buy
B: doesn't have to buy
C: needn't have bought
D: mustn't buy

In my opinion I think all of them are correct grammatically but just a different meaning.

I think the tense of the phrase is past simple tense so most people picked didn't have to.....so what you think?

The question is from second year high school exam in my country Egypt, kafr El shiekh governorate

1. She had extra groceries had home. She didn't have to buy any more.
2. She has extra groceries at home. She doesn't have to buy any more.
3. She had extra groceries at home. She needn't have bought any more.
4. She has extra groceries at home. She mustn't buy any more.

Note my corrections above.
Both A and C are possible and lead to grammatically correct sentences.
B doesn't work because "doesn't have to buy" doesn't collocate with "She had ...". The same goes for D.

The answer is supposed to be C (or B in the screenshot) but it isn't a valid question because the context doesn't rule out A.
