I found the sentence in a dictionary.

Is "a few miles" a noun used as adverb to modify "joins the river"?

I think "a few miles" could be seen as a noun phrase. It is, of course, indefinite. The person giving those directions might not know the exact distance, which is why "a few miles" was used.
