I told my friend I would take the IELTS last year but I haven’t taken it yet because I am always afraid that I can’t get the score I want and therefore I’ll be very upset. When he asked me why I haven’t taken the test, I said:

The thought that I can’t get the ideal score I want scares me.

Is the italic sentence natural?

I wouldn't use both "ideal" and "I want".

The thought that I won't get the score I want scares me.
The thought that I won't get my ideal score scares me.
