[General] The use od and/or when applicable

I'm in need of a little clarification for the following statement;

"Forms shall be identified with a title, form number, revision date and/or number when applicable"

Does the term "when applicable" refer to all terms listed, (title, form number, revision date) or does it only refer to the and/or number as stated?

Well, it's a bit confusing. I think it means it needs either a revision date or a revision number, or both if they both apply.
