I have added ... because she changes her mind- she starts saying you have to watch, then changes to say that you have to be careful what you eat.

He does murmur something, which sounds as if it ends in -ity, but I cannot catch what is said and I have played it many times.

1 When speaking, we often say things that would not work in writing. Speech is messy and often improvised- we chop, we change, etc. These things are simply characteristic of spoken English. You can't edit and delete things from speech, which you can do in writing, so it is inevitably going to be messier.
