A: I'm still on sick leave.

B: When do you have to return?

A: I haven't discussed that with my boss yet.


Is this a completely natural and neutral way to phrase the question or does 'have to' imply that B doesn't think that A wants to return?
 
Yes, it's fine. The 'have to' expresses that there is a duty to go back to work.
 
If somebody "has to" do something that means there is no choice in the matter.

A: I'm still on sick leave.
B: When are you going back?
A: Probably in a couple of days.
