Who can help me on my CV?

Dear teachers,
I would like to ask for your help. I wrote the attached CV for an international application, but I make a lot of mistakes. Could somebody of you help, please?

Maybe it would be better not to write it the way I did, but to write it in tabular form. What would you advice me to do?
