It means to be a born gardener
I have never used green-thumbed to describe someone good at gardening. I have only ever used green-fingered.
My grandmother was green-fingered and her garden was beautiful.

Green-fingered would sound as odd to us as green thumb sounds to you.
I'm not a teacher, but I write for a living. Please don't ask me about 2nd conditionals, but I'm a safe bet for what reads well in (American) English.
It means that someone is good at gardening or have talent in it.
Example: John is a green thumb, you can figure it out from his most beautiful garden in our town.
